Elevate - Creative Youth Space Food Order Form
Welcome to ELEVATE!
Hi Parents/Guardians,
Thank you for allowing your child/ren to join us at Elevate. We are having so much fun in rehearsals with your child/ren.
If you wish for your child to purchase dinner on the 26th of August please select what meal your child would like to order and submit this form no later than the 11th of August. Unfortunately, if your child/children do not submit this form before the 11th of August they will be unable to purchase their dinner and students will be required to bring their own food.
Your child/children are more than welcome to purchase more than one meal they will just need to pay for two (eg. x2 6pc nugget meals = $19.20).
Please also note that no parents will be allowed to drop food off to their children once we are at Parramatta Riverside Theatre.
