Here at Elevate, we are committed to providing the safest possible environment for your children. It is our goal to have up-to-date' best practice' Child Protection Policies in place to protect your children and our leaders. 

We, therefore, request your cooperation in this matter by completing the Registration/Medical form below.

As required by law all of our Elevate leaders over the age of 18 years have undergone the NSW Government Working With Children Check (WWCC). Our Elevate Leaders can be identified by the 'Elevate Leader' lanyards worn around their necks. In line with our current Child Protection Policy, each of our Elevate Leaders has undergone and completed our required Child Protection training course. From time to time we may have other adult assistants in attendance. Adults are able to assist in a limited number of areas up to five times per year without a WWCC. They can be identified by an 'Adult Assistant' lanyard.
